Here is a model of how the Long-Term Services and Supports (LTSS) website is formed and utilized:

The LTSS website’s next step is to find and train regional navigators who will work as a bridge to connect community members and tribal leaders with referrals to experts. 

Long-term services and supports embraces the continuum of care concept pictured below.
